Description
Pileus: hemispherical,campanulate; 10 – 20 mm diameter; radially fibrillose striate; ochraceous brown with ephemeral white veil remnants in horizontal bands which are denser at the top.
Stipe: central, cylindrical to slightly clavate; 20-40 × 5-7 mm; glabrous; white.
Gills: adnate; close; white maturing to black and deliquescing.
Flesh: white.
Spore print: black.

Notes: this campanulate ochraceous fibrillose fungus with distinctive white banded scales is usually found in small troops on logs and other woody substrates.
